Cancellation Policy

Waiver & Release of Liability By enrolling, attending, or participating in any activity with Practical Martial Arts, Inc. (“PMA”), I acknowledge and agree to the following: I understand that martial arts involve inherent risks that may result in serious bodily injury, death, illness (including illness from communicable diseases), and other physical, psychological, or economic harm (“Adverse Consequences”). I understand that these Adverse Consequences may result from my own actions or negligence, from the actions or negligence of others (including the Released Parties), or from the inherent nature of martial arts activities. I accept full responsibility for any injury, damage, or loss I may cause or suffer as a result of participation and agree to bear any related costs myself. I acknowledge that I am voluntarily assuming all risks associated with any medical or physical condition that I or my child may have, whether known or unknown, that could be aggravated by participation in martial arts. To the best of my knowledge, I have no communicable disease, infection, illness, virus, or bacteria that could pose a risk to others. I fully assume all risks and responsibility for any and all actions, inactions, or negligence on the part of the Released Parties. I voluntarily release, forever discharge, and agree to defend, indemnify, and hold harmless Practical Martial Arts, Inc., its owners, instructors, employees, agents, volunteers, and affiliates (collectively, “Released Parties”) from any and all claims, demands, or causes of action related to my participation in PMA activities, events, programs, or use of PMA equipment and facilities, whether arising from the negligence of the Released Parties or otherwise. This waiver and release expressly includes all claims alleging negligent acts or omissions by any Released Parties. I understand and agree that I will be responsible for payment of any attorney’s fees or costs incurred by the Released Parties in connection with any claim, demand, or lawsuit arising from or related to my participation in PMA activities.
